AEO and GEO are overlapping approaches to visibility in generated answers. The clearest practical distinction is the journey being measured: answer engine optimization focuses on direct, attributable answers in search features such as Google AI Overviews; generative engine optimization focuses on how ChatGPT and Gemini understand, cite and represent an organization across prompts.
AEO vs GEO at a glance
These terms are not governed by one industry standard, so providers sometimes use them differently. Rotgar uses the following working distinction to keep medical measurement clear:
| Question | AEO | GEO |
|---|---|---|
| Primary Rotgar surface | Google AI Overviews | ChatGPT and Gemini |
| Starting input | Search query | Prompt and possible follow-ups |
| Main output | Direct generated answer with cited pages | Synthesized conversational answer |
| Content emphasis | Extractable, attributable passages | Entity clarity, answer-ready content and source corroboration |
| Measurement | Overview presence, citations, competitors and sentiment per query | Mentions, citations, recommendations, competitors and sentiment per prompt |
| Shared foundation | Google Search quality, clear entities, evidence and accessible pages | Google Search quality, clear entities, evidence and accessible pages |
The table is a planning framework, not a claim that the two disciplines never cross.
Where do AEO and GEO overlap?
They overlap in most implementation work. Both benefit from technically accessible pages, coherent organization and clinician entities, direct answers, accurate structured data, visible authorship and credible supporting sources.
A strong medical service page can help Google understand a search query and give ChatGPT or Gemini a clearer source. One improvement may support both doors even though the results are measured separately.
Why keep the terms separate at all?
Separating the terms prevents unlike outcomes from being compressed into one vague AI score. A citation in a Google AI Overview attached to a search query is not the same observation as a recommendation after a sequence of prompts in ChatGPT.
The distinction also clarifies ownership. Google AI Overview work often begins with query mapping, organic landing pages and cited search results. ChatGPT and Gemini work may require a prompt journey, entity review and broader source analysis. Priorities become easier to explain when the evidence stays attached to the right system.
Does AEO replace SEO?
No. AEO depends on the Google Search foundation. Priority pages must be accessible, relevant, internally linked and supported. Answer-ready structure adds clarity; it does not bypass indexation, search intent or authority.
The full concept is explained in what answer engine optimization means.
Does GEO replace SEO?
No. GEO also builds on searchable, understandable and credible web information. A clinic with inconsistent services, unclear locations and thin pages does not solve those problems by adding the phrase “AI optimized.”
The broader definition is in what generative engine optimization means.
Do medical organizations need two separate strategies?
They need one coordinated search strategy with separate baselines and workstreams. The three-door model keeps traditional Google Search + Maps, Google AI Overviews, and ChatGPT + Gemini Visibility distinct while allowing shared technical, content and entity improvements.
That means the team can fix one underlying problem once, then assess its effect in the relevant systems. It does not mean selling the same deliverable twice under two acronyms.
Which term should appear in reporting?
Use the term that matches the evidence:
- use AEO or Google AI Overview visibility for query-level Overview findings;
- use GEO, LLM SEO or ChatGPT + Gemini visibility for prompt-level assistant findings;
- use AI search optimization for the combined program;
- define each term once and avoid switching labels simply to repeat keywords.
Clear reporting is more important than winning a terminology debate.
What neither AEO nor GEO can promise
Neither can guarantee a citation, recommendation or stable answer. Neither replaces clinical review, classic SEO or local visibility. Neither proves patient enquiries without separate business data. Both improve observable signals and use rechecks to understand whether those signals are changing.
