AI visibility is the extent to which generated search features and assistants represent an organization in their answers. For a medical organization, it includes whether Google AI Overviews, ChatGPT and Gemini name the clinic, use it as a source, present it as an option, omit it in favor of competitors or describe it inaccurately.
What are the main levels of AI visibility?
Presence inside an answer can have different meanings. Reporting every appearance as the same result hides the difference between awareness and a genuine provider-selection signal.

Competitor presence is the other side of the baseline. A question that names another clinic and omits yours identifies a different problem from a question where no provider is named.
Is AI visibility a ranking?
No. Traditional Google rankings assign pages positions for queries. Generated answers synthesize information and may name several sources or organizations without a stable numbered order.
This means “rank in ChatGPT” is convenient shorthand, not a conventional rank. Measurement should focus on observable answer features: presence, citation, recommendation, sentiment and competitors. The AI visibility score can summarize those observations, but should never replace them.
Where should AI visibility be measured?
Rotgar separates two generated-answer systems:
- Google AI Overviews: measured against priority search queries, alongside conventional Google Search results and cited pages.
- ChatGPT + Gemini: measured against priority prompts and relevant follow-up stages in a conversational journey.
Google Search + Maps remains a third, separate system with positions, impressions, clicks and local visibility. Together they form the three doors to patient discovery, but each requires its own baseline.
What should medical AI visibility cover?
The question set should be tailored to the organization’s actual services and markets. Useful coverage may include early treatment research, comparisons, specialist selection and local provider selection. The language should reflect how patients in that market ask questions rather than a literal translation of a generic English list.
There is no universal number of prompts that makes an audit complete. Coverage should expand or contract with the organization’s services, locations, countries and languages.
How is AI visibility measured over time?
Capture the exact query or prompt, platform, market, language and date. Record the answer, visible sources, competitors and sentiment. Re-run comparable tests after relevant changes and investigate unstable high-impact findings before setting priorities.

What can improve AI visibility?
The improvement depends on the observed gap. Common work includes:
- resolving crawlability or indexation barriers;
- clarifying organization, location, clinician and service entities;
- strengthening pages that answer priority questions;
- aligning visible content and structured data;
- improving authorship, review and source transparency;
- resolving contradictory external information;
- earning legitimate independent coverage where evidence exists.
The baseline should determine which action comes first. Publishing more content is not automatically the right response.
What AI visibility does not prove
Visibility does not prove a click, enquiry, appointment or revenue. Those outcomes require separate analytics or aggregated information from the clinic. Visibility is an upstream discovery signal that can help explain where relevant demand may be gained or lost.
It also does not guarantee future representation. The organization can improve the signals available to a platform; it cannot control the generated output.
